We have an unfinished basement with a finished concrete floor about 4' below grade. It is extremely cold in the winter. It is heated with duct outlets. What is the best method to insulate the floor. The house is built on gravel and there are no water problems.

The best thing you can do is provide return air ducts at floor level down there. A basement in particular should be serviced by a return air grill in every room. That's as close as you'll come to warming the place up. You could, of course, go to radiant heat in the floor but the costs of adding this can be something else.
